Why you should read this book

The authors bring the concept of Patient Safety to life, with real stories of medical error and near misses. Any Doctor will be shocked by the stories, but also think "There but for the grace of God, go I". Rather than just write "Work harder and be more careful" the authors give real pragmatic solutions that will help to reduce error and harm. The book is very easy to read, and does not require in depth academic knowledge of medicine, human factors or governance. I would make this mandatory reading for final year students.

All medical students should read this.
Any non clinician accountable for clinical care processes, Governance, incident reporting and investigation should read this book.
